Work Accident Notifications for Public Officers
Dear Personnel of Our University,
In accordance with the Occupational Health and Safety Law No. 6331:
" (1) The employer shall: a) Record all occupational accidents and occupational diseases, conduct the necessary investigations, and prepare the related reports.
b) Investigate incidents that occur at the workplace which, although not resulting in injury or death, cause damage to the workplace or work equipment, or have the potential to cause such damage, and prepare the related reports.
(2) The employer shall notify the Social Security Institution within the periods specified below:
a) Occupational accidents must be reported within three working days following the accident.
b) Occupational diseases reported by healthcare providers or workplace physicians must be notified within three working days from the date of notification.
(3) Workplace physicians or healthcare providers shall refer cases suspected of occupational disease to healthcare institutions authorized by the Social Security Institution.
(4) Healthcare providers must report occupational accidents they receive, and authorized healthcare providers must report diagnosed occupational diseases to the Social Security Institution within ten days at the latest.
(5) The procedures and principles regarding the implementation of this article are determined by the Ministry with the approval of the Ministry of Health." In accordance with these provisions, it is mandatory to ensure the recording and notification of occupational accidents and diseases.
Accordingly, in the event of an occupational accident in your unit, it is mandatory to immediately notify law enforcement authorities and report the incident to SGK within 3 working days via the civil servant occupational accident notification link. However, authorization is required to carry out this process. For the authorization process, the "Civil Servant Sick Leave Report Viewing and Occupational Accident-Occupational Disease Notification Application Authorization Request Form" available at the link must be filled out and submitted to the Social Security Provincial Directorates/Social Security Centers to which your unit is affiliated, in order to complete the authorization.
For employees under 4(a) status, the system must be accessed through "https://uyg.sgk.gov.tr/IsvBildirimFormu/gooKullaniciLogin.do". The notification must be completed in accordance with the principles outlined in the "Occupational Accident and Disease Notification Form Guide" available at the relevant link.
Together with the notification document submitted to SGK, the statements of eyewitnesses, photos of the incident site, if available video recordings, the occupational accident report, the incident site sketch, and the attached "Occupational Accident Report" and "Root Cause Analysis Form" must be completed, signed by the relevant parties, and submitted to our coordination office.
It is important to show due diligence to avoid administrative penalties by the Social Security Institution.
